Discipline templates

All templates start unverified, unassessed or BLOCKED. Null means unknown, not zero. Replace planning placeholders only with verified project facts and actual authorization; never fill them with plausible inventions.

The site-facts, task-intake, finding, handoff, architecture-decision, audit-coverage, publication-record, claim-ledger and independence-declaration files organize work. The release/publication contract and acceptance pairs are inputs to the strict offline evidence gate. A copied template is not a completed audit.

Evidence records use relative artifact paths inside a private evidence root, SHA-256, timestamps, build IDs and execution-run IDs. Preserve raw evidence privately and hash separately redacted public copies. Never include credentials or customer message contents in the library.
